A review is presented of the human contribution to accidents in high reliability industries. Previous research on human factors of accidents is given, three main areas being covered---organizational, job and personal factors. Consideration is also given to these factors in relation to forms for offshore oil industry accident reporting. Accident reduction strategies are also examined. 30 refs.
